Florida Information Sheet for Accessing Info from Sealed Adoption File

Chapter 63, Florida Statutes, governs the release of confidential information regarding adopted persons. If an adoptee (of legal age), and other parties to the adoption, are willing to sign consenting affidavits to release information, then we can issue pursuant to s. 63.162, (d)(a)(b0(c), F.S.

If a party to the adoption is deceased, proof of death must be provided in lieu of the authorization for release. Sample forms which you have the option of using are below for this purpose. should you choose not to use these forms, please ensure all the necessary information that is contained on the sample forms is also contained in your affidavits.

Fees required: $9.00 for search of our records plus $5.00 per document under seal.

If you are an adult adoptee and you are seeking identifying information regarding your original status and you do not have the authorization of the birth parent(s), section 63.162(7) F.S., provides that "The court may, at the request of an adult adoptee, for good cause shown, appoint an intermediary or a licensed child-placing agency to contact a birth parent who has not registered with the adoption registry pursuant to a. 63.185.F.S. to advise them of the availability of same".

However, court fees can be costly and you may first wish to consider registering with the Florida Adoption Reunion Registry, F.A.R.R.

to determine if a parent has already registered. If this has occurred, a match may be made and information may be released without having to incur the costs of court involvement. An order appointing an intermediary must come from the same court that the adoption was entered in. We can provide you with the court of jurisdiction and the county/state of the adoption upon receipt of a written requestaccompanied by picture ID or your signature notarized on the request.
